Quality and clinical governance

Adding an additional layer to the safety and effectiveness of our support, all personalised care packages are underpinned by robust clinical governance. This ensures that each individual’s care and support is overseen by an experienced Clinical Case Manager.

Our clinical governance framework also ensures that nurse-led training and mentoring is consistently delivered to our specialist Healthcare Assistants. Each Healthcare Assistant is trained by an expert Clinical Case Manager, tailored to the unique needs of the individual, whether a child, young person or adult.

Throughout the care journey, the assigned Clinical Case Manager will maintain regular contact to ensure individuals and their families are fully supported and continue to benefit from a high-quality, responsive service.

Continuous Improvement

We take a collaborative approach to delivering high-quality complex care and are committed to the continuous improvement of our services for children, young people and adults. In addition to maintaining best practice in line with our clinical governance framework, we actively seek feedback from individuals, families and their wider support networks. We also operate in a number of ways to reinforce a robust quality framework:

We continuously monitor and review clinical processes using the quality reflective cycle to improve our quality of care. We strive to be responsive to the needs of our clients through regular care plan updates, attendance of MDT meetings and package reviews.

To enhance the safety and effectiveness of our care and support, we manage clinical incidents and complaints, reporting directly to Integrated Care Boards (ICBs), Local Authorities, and other governing bodies.

We conduct regular audits and complete reports in key areas, to ensure care is of a high quality. We review how we are meeting the needs of our clients and their families, as well as how we manage incidents and complaints, and if our care plans are proficient and up to date.

We provide our Healthcare Assistants with expert training tailored to the individual needs of those they support, including children, young people and adults.

Our dedicated and experienced Clinical Case Managers provide continuous mentoring to each individual’s support team, ensuring care is delivered safely, consistently and in line with best practice.  New team members complete supervised shifts with Clinical Case Managers, who complete competency assessments before being signed off to deliver care.
